LIFE Centers Co-Located with senior living offers the opportunity to “age in place”
Trek has developed a space that co-locates a senior living community with LIFE Center to provide senior citizens with the ability to live comfortably in their own homes. Aging in place tends to improve seniors' quality of life, which improves their physical health. It also insulates them from the bacterial and viral risks found in senior living facilities, reducing their chance of contracting a serious illness. These senior housing communities offer 1-2 bedroom units designed around Health and Wellness centers to bring the CARE to you.
Community LIFE offers a wide range of services and care that are designed to keep seniors independent and in their homes. The LIFE program includes Health and Wellness Centers, medical care, in-home care, and transportation.
To be eligible, participants must be at least 55 years of age, clinically eligible, and able to live safely at home with the support of the LIFE program. Participants must also live in a Community LIFE service area. If you are eligible for Medicaid, there are no monthly premiums, deductibles, or copayments for any drug, service or care provided by your LIFE team.
